My husband and I cannot begin to express our appreciation, gratitude and love for everyone at Mosaic Gardens at Surprise. Today was my mother-in-laws first day at the facility. As she walked in, the staff and residents welcomed her with roses and a literal red carpet entrance. This whole process has been so positive! The staff alone make this place the BEST for your loved ones. Words cannot begin to express how thankful we are to everyone for not only making my mother-in-law feel home, but their support for our family as a whole!

Yesterday was my mom's first day at this wonderful memory care facility. The staff really made an effort to make her feel special and valued, with a red carpet entrance, a welcome sign, balloons, and flowers in her room. We can't wait to see how she connects with the staff and other residents and builds new friendships. Thank you, Mosaic Gardens at Surprise!

We moved my mom from another assisted living to Mosaic Gardens Memory Care, because the price was going up and the care was going down. When we my mom to Mosaic the care was wonderful they gave her a lot of attention. So great was the attention and care, that her health and attitude was better. They actually lowered the monthly payment to adjust for her increase in wellness because of their care. They were always had time to talk to me about my mom. I would highly recommend Mosaic Gardens Memory Care.

Mosaic and its predecessor have cared for my wife for the last year and a half. She is well cared for and appears to be doing well. The administration and staff are very supportive and are responsive to my requests. I am very pleased with Mosaic Gardens Memory Care.

What started as an excellent care facility has dramatically changed to very unprofessional, and in my personal experience has become a nightmare. The present director of nursing does not treat the residents, staff, or family members, with respect, from my experience her care plans look more like abuse and neglect. Warning be very careful about placing your loved ones in the Surprise Memory Care facility

This place has a revolving door of nurses. They use paper documents. Due to their old-fashioned record-keeping and revolving door of employees I seriously question the quality of care my love one week receives. The staff are very friendly, the only issues we've had with this facility is miscommunication, lack of accepting responsibility, lack of stability, and minor issues of unprofessionalism. For the amount of money we pay every month greatly disappointed the value and communication we receive. This facility is much nicer than Brookdale locations however operations need to be updated to provide your love one the care they deserve. Cindy the executive director has shown us that she does not have the best interest of the residents in mind. They make it extremely difficult for family and power of attorneys to get documents signed by their loved one in their care.

The staff members are great. The nurses and other staff work amazingly with the residents and really seem to care for them. The nursing staff and care staff know all the residents names and know what they need at any given time. The facility is clean and organized. Highly recommend someone who is looking for a place for a loved one, to cosider this place.

Avenir Memory Care in Surprise is absolutely beautiful inside and out. The staff are always going above and beyond to care for the residents and always communicate what is going on with my loved one.
